A one time membership card activation fee of $10.00 and first month’s dues MUST be paid upon sign-up.

Must have a completed and signed prescription form from physician.

All fees due before the seventh of each month and all annual memberships are due at the beginning of April each year.

You must pay monthly dues in order to keep your spot in this program, even if you are not participating. Three or more consecutive months of nonpayment will result in loss of your spot.

Medical holds will require written orders/statement from physician with specific date of return. no more than three consecutive months allowed for medical hold.

Cornerstone reserves the right to approve/disapprove any request to hold memberships.

A written letter resigning your membership or a Discontinue Membership Notice (ask for at the front desk) is required if you wish to drop membership.
